Large study finds genetic 'overlap' between schizophrenia, bipolar disorder

ScienceDaily (Sep. 21, 2011) ? Knowledge about the biological origin of diseases like schizophrenia, bipolar disorder and other psychiatric conditions is critical to improving diagnosis and treatment.

In an effort to push the field forward, three UCLA researchers, along with scientists from more than 20 countries, have been taking part in one of the largest collaborative efforts in psychiatry -- a genome-wide study involving more than 50,000 study participants aimed at identifying which genetic variants make people susceptible to psychiatric disease.

This collaborative, the Psychiatric Genome-Wide Association Study Consortium (PGC), now reports in the current online edition of the journal Nature Genetics that it has discovered that common genetic variants contribute to a person's risk of schizophrenia and bipolar disorder.

The PGC's studies provide new molecular evidence that 11 regions on the genome are strongly associated with these diseases, including six regions not previously observed. The researchers also found that several of these DNA variations contribute to both diseases.

The findings, the researchers say, represent a significant advance in understanding the causes of these chronic, severe and debilitating disorders.

The UCLA researchers who contributed to the schizophrenia study are Roel A. Ophoff, a professor of psychiatry and human genetics and one of the founding principal investigators of the schizophrenia portion of the study; Dr. Nelson Freimer, a professor of psychiatry and director of the Center for Neurobehavioral Genetics at the Semel Institute for Neuroscience and Human Behavior at UCLA; and Rita Cantor, a professor of psychiatry and human genetics.

Schizophrenia and bipolar disorder are common and often devastating brain disorders. Some of the most prominent symptoms of schizophrenia are persistent delusions, hallucinations and cognitive problems. Bipolar disorder is characterized by severe, episodic mood swings. Both affect about 1 percent of the world's population and usually strike in late adolescence or early adulthood.

Despite the availability of treatments, these illnesses are usually chronic, and patients' response to treatment is often incomplete, leading to prolonged disability and personal suffering. Family history, which reflects genetic inheritance, is a strong risk factor for both schizophrenia and bipolar disorder, and it has generally been assumed that dozens of genes, along with environmental factors, contribute to disease risk.

In the schizophrenia study, a total of seven locations on the genome were implicated in the disease, five of which had not been identified before. When similar data from the bipolar disorder study, which ran concurrently, were combined with results from the schizophrenia study, three gene locations were identified that proved to be involved in both disorders, suggesting a "genetic overlap" between schizophrenia and bipolar disorder.

"Genetic factors play an important role in the susceptibility to develop schizophrenia," Ophoff said, "but identifying these genetic factors has been very difficult. We know that schizophrenia is not caused by a single gene that explains everything but an interplay of many genetic and non-genetic factors."

At the same time, he said, the disease itself is not uniform but manifests itself in different ways; currently, there is no objective biological marker or "sign" that can be used for diagnosis.

"This so-called heterogeneity at the genetic and clinical level is the biggest challenge for genetic studies of neuropsychiatric disorders," Ophoff said. "One way to deal with these difficulties is to increase the size of the study so there is sufficient 'power' to detect genetic effects, even amidst this clinical and genetic diversity."

The fact that even this large study resulted in a limited number of schizophrenia and bipolar genes demonstrates once again, he said, the complex nature of the disease.

Formed in 2007, the PGC is the largest consortium ever in psychiatry. Over 250 researchers from more than 20 countries have come together in an unparalleled spirit of cooperation to advance knowledge of the genetic causes of mental illness. Crucial to the success of the project was the willingness of many groups to share genetic data from tens of thousands of patients collected over many years.

The research was funded by numerous European, American and Australian funding bodies. Funds for coordination of the consortium were provided by the National Institute of Mental Health in the U.S.

Google opens up, takes fight to Facebook (Reuters)

(Reuters) ? Google Inc and Facebook trotted out a variety of new social networking features in back-to-back announcements on Tuesday, underscoring their intensifying competition for Web surfers.

Google integrated its flagship search engine into its 3-month old social network -- with membership now open to the Internet public -- and expanded its "Hangouts" video-chat feature to allow mobile use and broadcasting.

The company said on its official blog its well-received Hangouts feature -- where up to nine people can link up and chat with a user on video -- will be available on camera equipped smartphones powered by its own Android software. Support for Apple Inc iOS devices "is coming soon", it added.

And a user can now host an online broadcast with this feature -- recording a session and broadcasting it live for public access online. Black Eyed Peas front man will.i.am will host the first "Hangout on Air" on Wednesday, Google said.

"Hangouts should keep pace with how you socialize in the real-world, so today we're launching it on the one device that's always by your side: your mobile phone," senior vice president of engineering Vic Gundotra said on the blog post.

For its part, Facebook said it was introducing a new "ticker" on its users' home pages, providing real-time notifications of what friends are doing on the service. Facebook also revamped the service's main news feed to flag important items -- such as a new baby announcement -- for Facebook users who have not logged on for a few days. Facebook also changed the way photos are displayed on the site, increasing the size of pictures that appear in a users' news feed.

Facebook is the world's No.1 social networking service, with more than 750 million users. The company has rolled out a series of improvements to its service recently, many of which seem designed to match features Google has used to set apart its rival social networking service, Google+.

Google did not say how many people had signed up for Google+ so far, but confirmed the social network was now open to all, whereas previously it had been invitation-only. Analysts estimate upward of 25 million users have joined Google+ since its inception.

The company also made its search engine available from within the social network. Users can search from Google+ and get results not just on the network, but from the worldwide Internet.

Google's infant social network, which counts Facebook CEO Mark Zuckerberg as a member, has met skepticism so far. Some are waiting to see if it can maintain the rapid momentum of its first months.

If CEO Larry Page's brainchild -- which some say mimics better than Facebook the instinctive categorizing of friends that occurs in real life -- takes off, it will come at a pivotal moment for its bigger rival. Facebook is widely expected to go public in 2012.

"We're nowhere near done, but with the improvements we've made so far we're ready to move from field trial to beta," Gundotra said.

WASHINGTON, D.C. Using breast magnetic resonance imaging (MRI) screenings among targeted, high-risk, underserved women significantly decreased diagnostic cost and increased patient compliance rates with follow-up compared to using general risk mammography screenings.

However, a caveat to these findings was that the cost of a MRI was reduced from an average of $3,500 to $649 by a grant specific to the study. Cost per diagnosis was $37,375 for mammography compared to $21,561 for MRI at the grant-based rate, according to the researchers.

"What we need is to lower the cost of MRI, and maybe that will happen as we do more of them," said lead researcher Anne C. Ford, M.D., assistant professor in obstetrics and gynecology at Duke University Medical Center.

Preliminary results of the study, conducted by Ford and colleagues from 2004 to 2011, were presented at the Fourth AACR Conference on The Science of Cancer Health Disparities, held in Washington, D.C., from Sept. 18-21, 2011.

The researchers compared breast cancer mammography screening in 299 general-risk, underserved women to MRI screening in 299 high-risk, underserved women. Women with abnormal mammogram or abnormal breast MRI underwent ultrasound, ultrasound guided biopsy and/or stereotactic biopsy for mammogram cases, and/or MRI guided biopsy for MRI cases.

Results showed that mammographic screenings detected one breast cancer case, while MRI screenings detected nine cases. Benign breast/total biopsies were found in 88 percent of mammographic screening cases and in 78 percent of MRI cases.

"In an underserved population, using this model, it is cost effective to screen with MRI because we found more breast cancers with MRI than we did with mammography in this population," Ford said. "If you truly target high-risk women with MRIs, you can find the cancers, and you can find them early."

In addition, compliance with follow-up in mammographic screenings was 75 percent and 90 percent in MRI screenings. Vital to those results was the utilization of a breast navigation team, which recruited study participants from the general population at health and screening fairs in central North Carolina, according to Ford.

"The navigation team was key in helping the women and these are all uninsured or under-insured women negotiate the medical center," she said.
